Disability and Carer Services

Commonwealth Respite and Carelink Centres –Free information about community aged care, disability and other services for carers. The program also provides emergency or crisis respite fro carers who are caring for a person who has a disability, chronic illness, mental illness, frail aged or with dementia. Nepean and South West Sydney

Young Carers – Support for young carers up to age 26 who are at risk of leaving secondary education because of their caring responsibilities. Nepean and South West Sydney

Mental Health Respite – Respite care for carers of people with severe mental illness/psychiatric disability. Nepean and South West Sydney

Support Co-ordination for Older Parent Carers –Support for older parent carers over the age of 65 who are caring for a son or daughter who has a disability. Nepean, Cumberland, Inner West and South West Sydney.

Compacks – A case management service for people being discharged from hospital and require two or more community support services. Penrith and Hawkesbury

Brokerage Options – This service provides case management for people with a disability to access community care and other community services. Blue Mountains and Hawkesbury

Community Options – A case management service for people with a disability and the frail aged who have complex care needs that cannot be met by existing community care services without case management. Penrith, Hawkesbury, Blacktown and Holroyd

Respite Options – A flexible package of respite care provided for people with a disability living with a carer. Nepean and Cumberland

Westlink Respite Services – A range of respite services for carers who have a child with a disability. Nepean and Cumberland

WHIRLS (Westlink Head Injury Recreation & Leisure Service) - A social and recreation group for adults aged 18 – 65 with an acquired brain injury. Blacktown
