We can assist with: My Aged Care related questions - Centrelink related questions - Interpreting letters from My Aged Care and Centrelink including letters regarding Income-tested Care Fee If you require a single CHSP service, these referrals can be handled by the My Aged Care team over the phone.
Undergo an assessment The ACAT or RAS member will make a time to come to your home to talk to you about your needs. They will also explain the assessment process. The assessment is free.
Receive the outcome of your assessment Home Care Package: You will receive a unique package referral code if you qualify for a Home Care Package. CHSP: You will receive a reference number for each particular service. Once you have either of these you can access Anglicare’s services under the Government subsidy.

Develop a care plan and enter a care agreement Once your package is released to you, or you proceed with our private services, our home care specialists can work with you to set lifestyle goals and create a personalised plan that helps you reach them. Once your personalised plan is ready, sign a care agreement with Anglicare and start receiving the services you require.
