From 1 July 2025, we'll provide this through the Support at Home Program, a program introduced by the Australian Government as part of the Aged Care Act 2024, designed to help older Australians live independently and safely in their own homes.

It brings together care and services into one simple, flexible program tailored to your individual needs — all possible with a little help from Anglicare.

Who is eligible for Support at Home?

An assessment for Support at Home is available to:

  • Individuals over the age of 65
  • Aboriginal and Torres Strait Islander people over the age of 50
  • Individuals over the age of 50 who are at risk of or experiencing homelessness

How do I get started with Support at Home?

To access Support at Home you'll need to complete an assessment process to determine the care you need and the level of funding you may be eligible for. This process uses the Integrated Assessment Tool, which looks at all areas of your daily life so you only need to tell your story once. This assessment is arranged via My Aged Care.

A trained assessor will have a conversation with you — either at home, online or on the phone — to understand the care you need. The assessor will ask questions regarding your health, lifestyle, mobility and daily activities. Based on this, you'll be assigned to 1 or 8 levels of support under the Support at Home program, which will determine the funding you can access.

You will also be assigned to a priority based on how urgently you need care.

From there, a care partner will develop a personalised care plan tailored to your needs and lifestyle to ensure you get the right support, when you need it. 

If I sign up for home care now, what happens when Support at Home begins on 1 July 2025?

If you start receiving home care now, you will continue to be supported through your Home Care Package as usual.

When Support at Home begins on 1 July 2025, you will transition to the new program without needing to reapply.


We will work closely with you to make sure your care continues smoothly, and that your funding and services align with the new system. You’ll move directly into the new Support at Home rules starting 1 July 2025, using a quarterly budget system with new funding arrangements.

Personal contribution fees will apply based on your income and the services you use. Some participants may pay less, others may pay more depending on individual circumstances. Speak to your care partner for more information.
