What is Case Management in Home Care?

 

Every individual is unique. You may have varying health / care needs as you age
or as your circumstances change due to different reasons.

Home care packages are available to assist you with services and care to help
you live as independently as possible in your home.

Organising home care services can be a difficult and time-consuming process.
This is why case management exists to ensure you can focus on what matters
most and get the care you need.

Through our case management service, we ensure that you always receive the
level of care that you need, when you need it and how often you require it.
We partner with you to manage your home care plan. We will work with you to
help identify needs, create and implement a personalised care strategy that is
built with your needs in mind, as well as supporting any existing carers or family
members in your life too.

Our experienced and compassionate case managers are responsible for these
plans. They work with you to build, advise, implement, and maintain (or change
plans, in consultation with you, when necessary) these plans.

What does a Case Manager do?

 

Your case manager's goal is to ensure that you can live as independently as possible. This may look like helping you to recover after a setback or health event and or reconnecting you with people and or pursuits to keep you connect and motivated.

Being relational in every interaction, with you, they almost become an extensive of your family. They get to know you; what motivates you, your likes, and dislikes. They also get to know your family, as well any other caregivers that may form part of your support network.

As part of getting to know you, they will determine the level of home care that's needed by understanding what activities you consider most important. This could be anything from social activities to medical appointments or hobbies. 

As part of re enablement, they will help you access Allied Health Services (Registered Nurses, Occupational Therapy, Physiotherapy, Speech Therapy, Dietitions) to provide assessments to support your health and wellbeing, assist with the application proccess for ACAT reassessments (as your care needs and in home support change) and any modifications around your home that need to be made.

levels/types of services. Knowing that your care needs are in experienced hands
gives you and your family peace of mind.
