Ben is a 35 year man who was referred to Anglicare by his father looking for extra support for his son. The referral requested assistance for Ben to attend medical appointments and encouragement around his daily living activities.
Ben has a diagnosis of Schizophrenia and has had a history of multiple hospital admissions in his twenties. Ben is now stable on medication but reports having difficulty with motivation and managing all the different aspects of his life. Ben also has diabetes and complex related health issues.
Ben engaged well with Anglicare staff and gradually started working on addressing his health issues. Anglicare staff referred Ben to a Psychologist and encouraged him to regularly attend his Psychiatrist and GP. Anglicare staff accompanied Ben to GP and he got a referral to the Endocrinologist. Ben gained skills around managing his diabetes and now takes his blood sugars regularly.
Anglicare staff also encouraged Ben to attend Independent Living Skills workshops and he completed the program, including topics such as lifestyle balance, cleaning, budgeting and self-care. Ben was able to practice the skills in his home with one-on-one tailored home visits. Ben also attends other activities provided by Anglicare including art group, cooking and nutrition classes and surf group.
Ben asked for support to put in an application for the National Disability Insurance Scheme. Anglicare staff were able to assist him through the process- including registering with the NDIA, liaising with his clinical supports to collect evidence and submitting his application. Ben was approved for an NDIS package and now has a higher level of support. Ben has chosen to use Anglicare to assist with support coordination and community access. Ben would like to use his package to build his independent living skills further with the goal of getting into volunteer work in the near future.
