Care finder
If you are in need of aged care services, but are having trouble understanding what you need to do and have no carer, friend or family member nearby to help, this free service is for you.
It is currently available in the Blue Mountains, Lithgow, New England North West, and the eastern suburbs of Sydney.
A care finder can help you
Anglicare’s compassionate and expert care finders can offer you the intensive support you need, at every step of your journey!
Step 1
Your care finder can contact My Aged Care on your behalf. They’ll arrange a care needs assessment for you to get the process started and support you when you are assessed.
Step 2
Your care finder can also shortlist aged care providers in your area, help you fill out your chosen provider’s forms and understand their service agreements.
Step 3
Once your supports are up and running, your Anglicare care finder will check in with you to make sure you’re happy with the care you are receiving.
Your care finder can also help you connect to other services you might need such as mental health support, housing support, drug and alcohol services and community groups.
Am I eligible for FREE care finder support?
The service is for vulnerable older persons who don’t have a carer, family member or friend to help them apply for care services. You might be eligible if you meet one or more of the following criteria:
- you have a cognitive impairment or difficulty processing information
- you are living in homelessness or at risk of living in homelessness
- you have trouble reading and writing
- English is your second language.
If you know someone who is resistant to receiving aged care support, they may also be eligible for this service.
